This puts Twitter service and Trust and Safety as well as anybody who cares about health on the platform in a very difficult position.

Somewhat. Somewhat they can make their own decisions. I say somewhat because they can't wake up one day and say "hey, I think I'll get married now. I'm ready." It doesn't work like that.
